  • Disclaimer: "I acknowledge that, in order to (watch this video, download this white paper, etc.), I agree to share my information with Company Name and receive occasional updates."

  • If the respondent does NOT check the box on the disclaimer, a message appears, "Sorry, your acceptance is necessary to continue."
